How It Works

Visit the website and choose a form type (Grievance, OSI, FOIL, DOCCS Contact, or Parole Grievance).

Fill in the details — the system maps your answers onto the official NY State form layout.

Submit. You receive a tracking code and can print a copy for your records.

Know Your Rights
- You have the right to file a grievance, and allegations of staff harassment or misconduct receive expedited review (7 NYCRR §701.8).
- Grievances must be filed within 21 calendar days of the incident (7 NYCRR §701.5).
- FOIL requests must receive a response within 5 business days (Public Officers Law §89(3)).
- You may appeal a denied grievance to the CORC within 7 days of the superintendent's decision.
- Anonymous OSI complaints are accepted. You may request confidentiality.
